George Mason University in Fairfax is accepting applications to fill a grant funded, restricted full-time Senior Community Coordinator position that will work with the Empowered Communities Opioid Project within the School of Nursing.
The Senior Community Coordinator is a peer support specialist who provides psychosocial assessments, crisis intervention, and resources referrals for individuals experiencing an acute behavioral health crisis. The incumbent will manage a caseload of recently released inmates who have an opioid use disorder and who need immediate behavioral health intervention. The Senior Community Coordinator collaborates with the participant, family, and care team to develop and implement the transitional care plan. Appropriate referrals are made to hospital inpatient settings, residential treatment centers, outpatient facilities, partial hospitalization programs, Intensive outpatient programs, outpatient clinics, community mental health centers, and private practice providers.
